The 'New REDCap Project Request Form' is for requesting new Projects to be hosted on the NHG-REDCap (Production) Server.

The Project Request Form is contained in a single ZIP file which contains:

  • Project Request Form (.docx)

  • Project Request Infosheet (.pdf)

Users are strongly encouraged to read the Infosheet document first before completing and submitting the Form.

  • We recommend that Users always download a fresh copy of the Form from this page to ensure they are always using the latest version.

  • Project Requests which are submitted on an outdated form version may be rejected.
