Project Closure Form
Use this Form to close Completed or Terminated REDCap Projects on the Production Server.

What this Form is used for
The 'REDCap Project Closure Form' is for closing REDCap Projects hosted on the NHG-REDCap Production Server.
Criteria for Project Closure
REDCap Projects can be CLOSED in the following situations:
(1) PROJECT COMPLETED
The Project is PERMANENTLY CLOSED to the enrollment of new participants and the addition of new data.
All the Project’s Activities, including Data Analysis, are FULLY COMPLETED. No further activity will be carried out.
The DSRB / IRB (or equivalent) has formally acknowledged the Project’s Completion.
(2) PROJECT TERMINATED
The Project has been TERMINATED / DISCONTINUED / WITHDRAWN before reaching its planned conclusion.
All activities have been stopped. No further activities will be carried out.
The DSRB / IRB (or equivalent) has formally acknowledged the Project’s Termination.
