Please download and complete the Project Request Form to request for a REDCap Project on the REDCap Production Server.

The file download is contained in a single ZIP file which contains the Project Request Form (.docx) and a Infosheet document (.pdf). Users are strongly encouraged to read the Infosheet document first.

We recommend that Users always download a fresh copy of the Form from this page to ensure they are always using the latest version.
